New Research Explores the Impact of Ketamine-Assisted Couples Therapy
A recent study has explored how combining a "relational dose" of ketamine with Imago Relationship Therapy (IRT) can affect couples' satisfaction and connection. Published in the Journal of Psychedelic Studies, the findings point to meaningful and lasting improvements in relationship satisfaction—some of which persisted for over a year after the treatment concluded.
What is a “Relational Dose”?
Instead of basing the ketamine dosage on a fixed weight-based formula, researchers tailored the experience to each participant’s subjective response. The goal was to reach a psychological state where individuals felt on the verge of deep inner exploration (sometimes described as a “k-hole”) but remained grounded enough to engage meaningfully with their partner during therapy.
Study Structure
The research employed a mixed-methods design involving 16 heterosexual couples who took part in a six-week group therapy program. Participants received weekly intramuscular injections of either ketamine or a placebo, followed by therapeutic activities guided by the principles of Imago Relationship Therapy. Relationship quality was measured before and after the intervention, and at intervals extending up to 15 months, using the Couples Satisfaction Index (CSI).
Qualitative Findings: A Relationship Reset
Many participants reported profound shifts in how they viewed and related to their partners. One couple described the process as hitting a "reset button," helping them let go of long-standing emotional baggage and reconnect with greater empathy and openness.

Data from the CSI supported the qualitative findings. Couples who received ketamine showed notable increases in satisfaction compared to the placebo group. On average, relationship satisfaction rose from a baseline score of 109.08 to 126.17 by the final follow-up, which occurred between 1.5 and 15 months post-treatment (averaging around six months). These results suggest not only that the positive effects lasted but also that relationship satisfaction continued to grow over time.

While the sample size was modest, the study offers promising insight into the integration of psychedelic experiences with couples therapy. It opens the door for future research to explore ketamine’s potential in fostering emotional reconnection and healing in relationships.
Further studies with larger, more diverse populations will be crucial to deepen our understanding of the long-term impacts and therapeutic mechanisms involved. Still, this pioneering research signals an exciting development in the use of psychedelic-assisted therapy to support couples in transforming and revitalizing their relationships.
